Practice Fees
Our practice follows the guidelines recommended
by the American Medical Association and the Resource Based Relative Value System.
This system determines the value and expense
components of each servie and assigns a unit worth (value). Our fee schedule is designed
according to these guidelines. If you have a question regarding a service fee, please make
an inquiry.
Accepted methods of payment are cash, check, money order, Mastercard
or Visa. Full payment of the balance for which you are responsible is due at the time of the visit unless special
payment arrangements have been approved.

Insurance Billing
Our medical practice has a contractual relationship with
multiple insurance carriers.
This list changes periodically so please check with our
staff regarding your company sponsored insurance plan.
If you have coverage through one of our carriers you will
be asked to pay your co-pay amount at the time of your appointment and our office will bill your carrier
for the rendered services.
For insurance carriers that we do not participate with, you
will be asked to pay for all services at the time of your appointment. As a courtesy, we will bill your
insurance and instruct them to directly reimburse you.
