What will my bill include?
Bills include professional service fees for care rendered by
either a physician or nurse practitioner. Also included are injections, medications. clinical tests, ultrasound,
mammograms, physician surgical fees and most lab work.
Who do I call if I have questions?
For billing questions or to give insurance information, please call
our Patient Billing Department Monday through Friday. If you are covered by more than one insurance company,
please notify the billing department 428-2800 ext 40 or ext 25.
What if I cannot pay my balance in full?
If you cannot pay in full at the time of the service, please call the
Billing Office at 428-2800 ext 40 or ext 25, to help you with making payment arrangements. We will be able to grant
you a short term extension.
What if I have no insurance coverage?
If you find yourself without insurance coverage due to loss of coverage,
delay at your place of employment or for any reason, we want you to know that we share your concern. We have designed a
cash discount policy to assist you during those special times. You only need to advise our staff as you check-in at your
appointment and then when you check-out remind the receptionist to give the special cash discount policy. This will
price all your services at the lowest fee allowable. In order to take advantage of this special policy, you will need to
pay in full at the time of the visit. If you request a delayed payment plan, as described above, the standard fees will be
assessed.
